Message First off, I don't blame new employees, everyones got to start somewhere. That being said, is there a post production inspection that the pipes go through before being sent out? If not and the pipe is inspected by the person who welded it, (and I'm sure that indiviual believes he did his best) Is there either a slip of paper with an "inspector #" OR a way for them to stamp their initials on the outside corner so it is well known who screwed up???
My thoughts behind stamping it (although employees come and go) is not for the fact of saying, you screwed up, you're fired... but to bring it back to the source and have the individual who made it learn from his mistakes.
This is another form of feedback that should make it back to the person that makes that specific DP so they know where it failed, why, and how to improve.
